The forms below cover everything we need for your first visit — medical history, insurance information, consent to treat, and HIPAA acknowledgment.
- Patient registration — your contact info, emergency contact, and insurance details.
- Medical & dental history — current medications, allergies, recent procedures, and any health conditions we should know about.
- Consent to treat — standard authorization for routine dental care.
- HIPAA notice acknowledgment — required by federal law (we’ll explain it at check-in if you have questions).
HIPAA note: Our online forms use a HIPAA-compliant third-party service. Forms submitted through this site are encrypted in transit and at rest, and only authorized practice staff can access them.
